What to avoid — MB safety reference

Asymmetry rule: a flare from accidental aged-cheese is uncomfortable. A hypertensive crisis from accidental DXM-cough-medicine while on MB can be life-threatening. The list below is asymmetric on purpose — over-cautious for the high-stakes items.

🆘 If you accidentally consume a NEVER item — call 112 immediately

Serotonin syndrome signs (5-HTP/SSRI/DXM/Tramadol/etc. + MB) — onset within 1-12h post-exposure:

High fever (>38.5°C), profuse sweating
Muscle rigidity, twitching, hyperreflexia, clonus (rhythmic jerks)
Agitation, confusion, hallucinations
Rapid heart rate >120 bpm, BP swings, dilated pupils

Hypertensive crisis signs (tyramine/pseudoephedrine + MB) — onset within 30 min — 2h post-exposure:

Sudden severe headache ("thunderclap" or worst-of-life)
Chest pain, sweating, palpitations
Stiff neck, nausea/vomiting
Vision changes, lightheadedness

Either set of symptoms → call 112. Don't wait. Tell them "Methylene blue + [substance]" so they can rule out / treat correctly.
